Individuals who request documents.
Data regarding the request for copies of annual financial reports,
information reports, and constitution and bylaws filed with the Department of
Labor by labor unions in accordance with the public disclosure provisions of
the Labor-Management Reporting and Disclosure Act of 1959, as amended (LMRDA).
Data includes individual requester's name, title (optional), organization
(optional), street address, city, state, zip code, telephone number (optional),
fax number (optional), e-mail address (optional), user name, and password; type
of request (walk-in, telephone, mail, Internet, or fax); date of request;
copying and certification charges; name and amount on requester's check; the
name and LM Number of the labor union for which information has been requested;
and the documents requested.

These records are used by authorized OLMS disclosure personnel to
process requests made to the OLMS Public Disclosure Room, prepare requests for
payments, and process payments.

Prefatory Statement to this document.

By individual name, organization name, address, control number, or
request date.
Computer system is password protected and accessible only to personnel
creating and maintaining the database.
The data is deleted after one year or when no longer needed, whichever
is later.

Individuals wishing to inquire whether this system of records contains
information about them should contact the system manager indicated above.
Individuals must furnish their full names for their records to be located and
identified.
Same as notification procedure. Individuals requesting access must also
comply with U. S. Department of Labor Privacy Act regulations on verification
of identity at 29 CFR 71.2.
Same as notification procedure above except individuals desiring to
contest or amend information maintained in the system should direct their
written request to the System Manager listed above, and state clearly and
concisely what information is being contested, the reasons for contesting it,
and the proposed amendment to the information sought pursuant to 29 CFR
71.9.
