KDFWR – REPLACEMENT LICENSE FORM

Replacement licenses are available only through the KY Dept. of Fish & Wildlife office in Frankfort. There is a $5.00 replacement fee on all licenses and permits. If you want to hunt or fish before the replacement arrives, you must buy a second license and request a refund. For a refund, you must give us the number on the second license. You will be refunded the cost of the second license less the $5.00 fee.
