How to Get OSHA Student Course Completion Cards

After conducting an outreach training course, submit documentation about the course to the appropriate organization, as outlined in Attachment B of the program guidelines. Your documentation will be reviewed and OSHA course completion cards will be sent to you for your students.

Your documentation must include the following items.
  • OSHA Outreach Training Program Report which includes your signed Statement of Certification
  • Legible list of Student Names
  • Topic Outline which lists the topics covered in the course and the amount of time spent on each topic. You can use the OSHA Outreach Program Report to provide this information for 10-hour course.
  • Copy of Trainer card if this is your first time requesting cards, or if your trainer status has changed.
Retain the following records of your classes for five years:
  • Student sign-in sheets for each class day
  • Student addresses
  • Copy of the documentation sent in to request cards
  • Records which indicate the card number dispensed to each student
After receiving your request, it may take two to four weeks to process your request.
