1) Complete the application form online
Business Service Provicer Application Checklist:
o Complete application on-line at: http://www.buyusa.gov/baltimore/bspapplication.html
o Complete payment form and fax to (410) 962-4529.
o Send company logo (jpg file)to Jolanta.Coffey@mail.doc.gov
(File must be no larger than 200 pixels wide for upload).
o Sign and FAX completed Participation Agreement (you will receive this via email after your application and payment is submitted).
