If you purchased a Horizon Life policy from American-Amicable Life Insurance Company of Texas or one of its affiliates between January 1, 2000 and July 28, 2006, and you were in the military when you purchased the policy, you may be eligible to receive cash compensation from the company. If you are eligible, there is no paperwork you need to fill out. In October 2006, American-Amicable mailed notices and checks to all eligible service members at their last known addresses. If you did not receive a check but believe you should have, you can call the company toll-free at 1-800-736-7311.
