Help with Waiver Request
It's easy to enter your waiver request, by just clicking on Waiver Request.

Waiver Request

To access our Waivers web application, the "Log In / Registration" web page will require a valid user id and password. Here are some steps that you can take to login:
Login Step 1.
Enter your UserID and password, and then click the "Login" button.
Login Step 2.
If you are cannot remember your UserID and/or password OR if you are converting your old e-mail-based login to the new UserID-based login then use the 'reset account' feature.  Enter your E-mail address and click the 'Reset Account' button - we will send you a new UserID and password.
Login Step 3.
If you do not already have a UNICOR online account, please create one by typing your e-mail address and clicking the 'Register Now' button.

Help with "Waiver Request"

Waiver Request will allow you to enter your UNICOR Waiver Request online.

* Required Information

The following waiver information is needed:
Department of Defense $3,000 Program
Customer Reference Number
*Justification
The following line item information is needed:

*Description

*Quantity

*Unit Price

If your waiver request contains more than 10 items, please fax your waiver request to
UNICOR Customer Service Center at (859) 254-9390.

The waiver request process will involve 2 steps.
1.
Data Entry.
Fill-in a blank waiver request form. After the form is completed, you need to click the "Submit Waiver Request" button. If your waiver request has been validated successfully, a Review Waiver Request web page will appear.
2.
Review.
After you review your waiver request, you need to click the "I Agree" button. This will store your waiver request into our waiver request database and give you a UNICOR Waiver Request Number.
