Kaltech Industries Waste Mixing Explosion

Summary
The CSB investigated an explosion at Kaltech, a sign manufacturing company in New York City. The investigation found that Kaltech failed to maintain an OSHA-required hazard communication program and failed to manage hazardous waste in accordance with established EPA regulations. Investigators also noted that New York City fire codes do not incorporate important hazardous chemical safety requirements, and federal, state, and local authorities failed to inspect the facility. The CSB issued safety recommendations based on these root and contributing causes.
