Summary
The CSB investigated a vapor cloud fire at the BLSR Operating Ltd. oilfield waste disposal facility near Rosharon, TX. The investigation found that Noble Energy failed to identify and communicate the hazards associated with basic sediment and water (BS&W) and that the company did not provide hazard information to vacuum truck drivers. In addition, BLSR management did not have effective hazard communication practices in place, nor did it implement safe handling practices when offloading flammable liquid. The CSB issued safety recommendations based on these root causes.
