‡ Self-reported by provider. Public provider includes public health clinics and community health centers. Private provider includes
private clinics, HMOs and group practices. Mixed provider includes more than one type of provider. Other provider includes
all other types of providers such as hospitals, military facilities, and unknown responses.
Beginning in the third quarter of 2007, further efforts to identify self-reported provider facility type classified as "unknown" resulted in
decreases in the estimated percentage of children in "other" provider facility type, which is comprised mostly of hospitals, military and
unknown facilities, and increases in the estimated percentage of children in known provider facility type (e.g, public, private, and mixed)."
§ Children in the Q3/2007-Q2/2008 National Immunization Survey were born between July 2004 and January 2007.
