Figure 2: Chronic Care Model

The chronic care model has six components that include both the community and the heath system. They are: community resources and policies, health care organization, self-management support, delivery system design, decision support, and clinical information systems. These components lead to productive interactions between an informed, activated patient and a prepared, proactive practice team. These result in improved outcomes.
