Downloading, Installing, and Using the ICS Forms
Installing and running the ICS forms database
On a Windows 2000/XP PC
Click the download link (at page bottom). When prompted to save the file, click Save File to download the installer. Next, double-click the installer and follow the directions you'll see on your screen to install the database.
To start the database, click the Start menu, point to All Programs, then choose ICS Forms.
On a Macintosh
Click the link (at page bottom) to download the database. You will see the disk image file, ICS_DB.dmg, on your desktop. This file will automatically open to show the ICS Forms Folder. Move the ICS Forms Folder to wherever you would like your ICS Forms database to reside.
To start the database: Open the ICS Forms Folder, then double-click the file, ICS Forms.
Installing and using the ICS Forms PDF set
On a Windows 2000/XP PC

To view or print any of the ICS forms, either double-click on the icon for any of the files, or open any of the files in Adobe Reader (link at right).
On a Macintosh
The PDF set has been compressed as a single file so that you can download it from the Internet. Most browsers can automatically decode compressed files. If yours can, once you've clicked the link to download the file, you should see a folder called "ICS_PDFs" on your desktop. This folder contains the ICS Forms PDF set. (If your browser can't decode files, you'll need a decoding utility like Stuffit Expander (link at right). To uncompress the file, use your decoding utility. If you are using StuffIt Expander, either (1) drag the file onto the Stuffit Expander icon on your desktop, or (2) start Stuffit Expander, then from its File menu, choose Expand, then select the file.)
