Sentinels of Safety Verification Letter


Description: All eligible mine operators who would like to be considered for the Sentinels of Safety Awards Program must return a Sentinels of Safety Verification Letter. The Verification Letter is sent out to each eligible mine operator and indicates the hours worked and No Days Lost Injuries in the portion of property being considered for the award. The letter confirms the accuracy of the information MSHA has on file for the operation in question. Changes cannot be made online if the hours or injuries MSHA has on file are not correct. The Verification Letter, along with amended 7000-1 or 7000-2 forms, must be send to the fax number or mailing address listed below. A copy of the Sentinels of Safety rules can be found at http://www.msha.gov/SentinelsofSafety/SentinelsofSafetyrules.asp and eligibility for the awards program can be found at on the MSHA homepage at http://www.msha.gov/SentinelsofSafety/SentinelsofSafetyhomepage.asp.
Filing Options: Eligible operators can submit their Sentinels of Safety Verification letter online or can return a paper version of their letter by Toll Free Fax to 1-888-231-5515 or by mail to:
