SUBJECT: Fatality Review Committee Membership

The purpose of this memorandum is to revise the membership of the Fatality Review Committee. Effective immediately, Jeffrey Duncan, Director of Educational Policy and Development, will serve as Committee Chair. In addition, Syed Hafeez and Kenneth Bullock, Acting Director and Acting Deputy Director of PEIR respectively, will now serve on the Committee. All other duties and responsibilities of Committee members remain the same. The Fatality Review Committee now has the following members:
  1. Director or Deputy Director of Educational Policy and Development (Committee Chair)
  2. Acting Director or Acting Deputy Director of PEIR
  3. Director or Deputy Director of Technical Support
  4. Associate or Deputy Associate Solicitor for MSHA
  5. Senior Program Management Consultant, Clinical Operations, Federal Occupational Health Service, United States Public Health Service
