Part 46 Training Forms
Part 46 Training Assistance - Forms


MSHA offers the "MSHA Training Plan Advisor". This on-line compliance tool will help you with developing both a Part 46 or Part 48 training plan. We recommend that you start there. To access this application, please use this link.


This is the menu page for the forms found in the Part 46 Starter Kit. You may fill these out while on they are on your computer screen or download these forms by using the "File Save" option in your browser.

Please be aware that on-screen completion of forms is not to be confused with on-line submission. After completing your selection, you will need to print the plan on your printer.

You may also choose to submit your training plans to MSHA via regular mail or e-mail. If you wish to submit your plan by e-mail, it will need to be in Word format. PDF forms will not save the data when you save the form. If you have Adobe Acrobat you may export data to a form data file and then import that data back into the same form. This gets complicated for alot of users. We are working on alternative methods for submitting this data. You will notice that there are buttons at the bottom of each page designed to clear a particular page of information. There are also buttons on the last page of each document which will allow you to clear the whole form or print the form.

Important: You must have Adobe Acrobat Reader 5.0 or higher for these forms to function properly. If you do not have this version, it may be downloaded from Adobe, free of charge. Please use the link below to download the latest version of Adobe Reader.
