The CDC recommends routine rather than risk-based HIV testing.  Limitations of risk assessments and the benefits of early diagnosis and treatment have led the CDC to recommend all persons between the ages of 13-64 be tested.  If a patient is also deemed to be of high risk, he/she should be tested at least every year.  If a patient has no known risk factors, the test should be given every 3-5 years.

Routine HIV testing is deemed cost effective if the prevalence is greater than 0.1%.  If a community does not know its HIV prevalence, it should initiate testing until it can establish it is under the threshold of 1 in 1000 individuals being HIV+.
