Maternity/New Child

To enroll your child in benefits, you must contact the Benefit Operations Team with 31 Days of your child's date of birth (or adoption). Proof of the event is required (birth certificate or hospital document). Follow these steps if you are pregnant or have a new child:

Step 1 Before or early in your pregnancy, request a confidential consultation and workplace evaluation from the Reproductive Health Assistance Program (RHAP) Team. This will identify possible risks to you and the fetus in your workplace and at home.

If you are enrolled in a UHC Medical Plan, please consider signing up for the Healthy Pregnancy Program (there is no additional fee for this service). Information can be found here . To schedule your confidential pregnancy counseling appointment, call the Lab’s Occupational Medicine Group at 505-667-7839. After this initial appointment, you will be scheduled with the two other groups comprising the RHAP team: Radiation Protection Services and Industrial Hygiene and Safety. These groups will provide you with information about a healthy pregnancy.
Step 2 Review the Laboratory Policy on the Family and Medical Leave (FMLA) P730-13 (pdf). If you meet the qualifications for FMLA, complete the appropriate FMLA forms. If you have questions or need assistance, contact a FMLA consultant in HSR-2 at 505-665-2620. .
Step 3 If you plan to access your disability insurance during this time, schedule an appointment with the Lab’s Disability Coordinator at 505-667-1203 for more information. It is recommended that you call in the early part of your third trimester. During your appointment, your current benefits coverage will be reviewed and your disability options discussed.
Step 4 Check out the Wellness Center Healthy Start health promotion class.
Step 5 Add your new child to your insurance plans by contacting the Lab's Benefits Office at 505-667-1806. You have 31 days from the birth/adoption to make additions or changes to your eligible benefits.
