FY 2009 CHGME Annual Report:  Helpful Reminders

The CHGME Annual Report Forms and Guidance are posted on the CHGME website under the tab “Annual Report
The FY2009 Annual Report requires information about the 2007-2008 academic year.

Your hospital’s completed, signed annual report package must be postmarked by February 9, 2009 and submitted to the:

The Annual Report submitted to HRSA should include:

  • one (1) paper copy of completed Annual Report with the required signed certification
  • one (1) electronic copy of the completed Annual Report on CD. 
  • one (1) electronic copy of the Discharge Data by Zip Code on CD

Some important specific reminders  for completing the forms:

    • HRSA 100-1-B.  At the top of the page don’t forget to include the number of outside institutions that sent residents to your hospital in the 2007-2008 academic year.  This information is important for showing how CHGME hospitals are serving other hospitals and medical schools.  The number should include all institutions, even those sending residents for brief rotations.
    • For the hospital-level information provided in the HRSA 100-2, check the number of discharges reported for the academic year.  The figure should be similar, but different, from the number of discharges reported in your application for funds.  If there are gross differences, check for errors and send a file with non-missing information.
    • It is very important to include the city, state, and zip code for the discharges.  If these data are missing, we cannot determine the extent to which discharges come from underserved areas.
    • On Form HRSA 100-2-D, “area addressed” refers to whether the program was offered in the 2007-2008 academic year—check for yes, blank for no.
    • On Form HRSA 100-3-F, it is also very important not to have missing data for city, state, and zip code for graduates.  These data are used to identify underserved areas. 
    • For those items that ask for a written response regarding changes and benefits, please link the reasons to residency training.
