Annual Conference
The annual TB Education and Training Network (TB ETN) conference
emphasizes the steps of the systematic health education process:
needs assessment, development, pilot-testing, implementation, and
assessing effectiveness. Conference activities include skills-based
workshops, informational presentations, and networking
opportunities.
The Ninth Annual TB ETN Conference, titled “TB Education and
Training: Recipes for Success”, will be held July 28-30, 2009 in

There is a registration
fee for this year’s conference. Your conference registration will
not be final until both the registration form and payment form have
been received. You will receive notification by e-mail once your
registration is complete.

Registration
Fees
The registration fees are in U.S. dollars as follows:
- TB ETN members: $50.00
- Non-members: $75.00
Payment is due to the American Lung Association Serving Georgia
and South Carolina at the time of registration and is accepted in
the form of credit card only (VISA, Master Card, and Discover).
Registrations are considered pending until payment is received.
Onsite registration will not be accepted. Do NOT send
registration fee payment forms to CDC!
**Scholarships are not available for registration
fees or travel to the conference.**

Registration fee payment forms received by the registration
deadline (July 7, 2009) will be confirmed by either e-mail or fax.
This registration confirmation should be used as your payment
receipt. Contact Pam Collins by e-mail (pcollins@alase.org)
or phone (1-800-LUNG-USA, EXT. 521) if you do not receive a
confirmation.
Your conference registration will not be final until both the
registration form and payment have been received. You will receive

Cancellations must be sent to Pam Collins of the American Lung
Association Serving Georgia and South Carolina in writing by e-mail
(pcollins@alase.org) or fax
(770-319-0349). Telephone cancellations will not be accepted.
Full refunds will be provided if cancellation is received by July
7, 2009. No refunds will be given after this date. Refunds will not
be granted for failure to attend, late arrivals, or early
departures.
Substitutes may be sent in the event that the registrant cannot
attend the conference. Substitutes must complete a conference
registration form for their registration to be complete.

Please submit your conference registration form and payment form as
soon as possible, but no later than July 7, 2009.
The Ninth Annual TB ETN conference will be held at the
Westin Atlanta North at Perimeter located at 7 Concourse Parkway
in Atlanta, Georgia, 30328.
Attendees are responsible for making their own reservations with
the hotel by calling 1-888-627-8407. Guest rooms are being offered
at the current government rate of $141 per night. Be sure to mention
the group name, CDC – TB Education and Training Network 9th
Annual Conference, to receive the government rate.
Hotel reservations must be made by July 6, 2009 to secure
the government room rate.
An early departure fee of $75 will be charged to a guest’s
individual account if the guest checks out prior to the reserved
check out date. To avoid this fee, advise the hotel at or before
check-in of any change in the scheduled length of stay. Guests will
be reminded of this fee upon check-in.
The Centers for Disease Control and Prevention is accredited as
a provider of Continuing Nursing Education by the American Nurses
Credentialing Center's Commission on Accreditation.
