Instructions

 

1.     Complete the Electronic Application Filing Request. [To determine if an application is delegated (may be acted on at the Regions) or non-delegated (must be acted on by OTS Washington), please refer to the linked guidance.]
2.     Upon receipt of the form, OTS may need to contact you.
3.     You will receive a secure e-mail from OTS containing the identification number for your application.
4.      Retrieve the secure e-mail, attach your completed application and related files to the e-mail, and reply securely to the OTS. The submission must include a scanned copy of the signature page. You must clearly mark portions of your application you consider confidential and non-confidential (12 CFR Section 516.35) and submit the confidential portion as a separate attachment. You must submit the application form in Portable Document Format (PDF). Click here to download Adobe Reader. Application filings must not contain security settings and scanned images must be searchable. Refer to OTS Secure E-mail Procedures.
5.     You must retain the original manually signed signature page for each signature that appears on the scanned signature page(s) within an electronic filing. You must make this document available to OTS upon request for a period of five years.
6.     OTS will issue a confirmation e-mail to acknowledge that your filing has been received. If you do not receive this confirmation, please contact the appropriate party at OTS. You may use the identification number to inquire about the status of your submission to OTS.
 
Please note, that an electronically filed application or notice will not be considered accepted for review purposes until OTS is in receipt of the applicable filing fee(s). 
Fees related to filings can be submitted by wire transfer, using instructions available upon request when the application is submitted via secure messaging.
