NOAA Form 370
The NOAA Form 370, Fisheries Certificate of
Origin, is required to accompany all imports of
frozen and/or processed tuna
products and is to be submitted to U.S.
Customs and Border Protection (CBP) at the time of importation. It is
important that all NOAA Form 370s be filled out properly and completely.
In particular, Section 1 of the form asks for the U.S. Customs entry
number. This piece of information should be readily available from the
U.S. customs broker handling the entry. Incomplete forms may lead to a
delay or rejection of the filed paperwork by CBP.
Effective
February 12, 2009,
the importer of record must within ten
days of the shipment's entry submit a copy of the Fisheries
Certificate of Origin and associated certifications to the Tuna Tracking
and Verification Program (74
FR 8, January 13,
2009, pp. 1607-1620).

In addition, 64 FR 122, June 25, 1999, pp.
34217-34218 identifies Italy as a Large Scale High Seas Driftnet nation. Import
shipments from Italy containing fish and fish products specified in regulations at 50 CFR 216.24(f)(2)(iii) must also be accompanied by
a NOAA Form 370, and an official of the government of Italy must sign and date the high
seas driftnet certification on the form (Box 7) to certify that the fish or fish products
were not harvested using a large scale driftnet on the high seas.
