Obtaining Blank Federal Tax Deposit Coupons |

The IRS is frequently asked, “Why can’t I get a blank Federal Tax Deposit (FTD) Coupon, Form 8109-B?” The answer is, under certain circumstances, blank coupons are available. However, blank coupons are provided only when absolutely necessary.
The IRS policy for issuing Forms 8109-B was developed in response to concerns over disclosure of taxpayer information and the creation of erroneous tax deposit accounts. The current policy also serves to counter identity theft, which has become more prevalent in the business community.
Therefore, we are required to verify the individual requesting Forms 8109-B is an authorized individual acting on behalf of the business entity. This individual can be the taxpayer, an officer of the company or an individual authorized to pay or deposit taxes, or an authorized individual with a valid power of attorney on file with the IRS. This applies whether the Form 8109-B is requested at a Taxpayer Assistance Center or via the toll-free business line (1-800-829-4933).
Local Taxpayer Assistance Centers can provide an authorized individual with two to eight Forms 8109-B with the Employer Identification Number and business information already entered, depending on the type of tax form involved and the required deposit frequency.
