Lufkin, TX - Citizens who sustained property damage due to the breakup of the Space Shuttle Columbia and plan to seek reimbursement may file a damage claim with one of three offices of the National Aeronautics and Space Administration (NASA).

Claims should be filed using U.S. Government Standard Form 95, "Claim for Damage, Injury or Death," which can be obtained from the NASA chief counsel's office at one of the facilities listed below.

NASA will have inspectors in the field to verify claims. Claimants can expect to have their property inspected and should give a phone number and address where they can easily be reached. The form is self-explanatory. If possible, supporting statements, photographs, and estimates should be included.

Generally, the person who suffered the damage, loss, or injury or that person's representative must file the claim. Some citizens have called FEMA seeking to register damage claims. These calls will be referred to NASA.
