The purpose of this Notice is to announce the NHLBI's intention to issue a Request for Applications (RFA) in the fall of 2005 to fund five-year randomized controlled trials (RCTs) to test the effectiveness of interventions delivered in routine clinical practice on achieving weight loss in obese patients who have other cardiovascular risk factors (CVRF). The receipt date for applications would be in the late winter of 2006.
Effectiveness of weight-loss interventions in routine clinical practice are the primary focus. The interventions should be based on prior evidence, be 2 years in duration, and have patient BMI as the primary outcome. Although weight-loss interventions are the primary focus, an important secondary focus of these effectiveness clinical trials is to incorporate the weight loss strategies with approaches to improve physician and patient adherence to evidence-based clinical practice guidelines to reduce the other CVRFs commonly present in obese patients, such as elevated LDL-cholesterol, hypertension, diabetes, or cigarette smoking. The investigators may select obese patients with only one specific additional other CVRF (e.g., focused on obese patients with hypertension) or obese patients with any one of the other, or with multiple other, additional CVRFs. Approaches directed at changing provider and patient adherence to weight loss interventions may also be used to improve provider and patient adherence to behavioral and pharmacotherapy components of the clinical practice guidelines for the other CVRFs (http://www.nhlbi.nih.gov/guidelines/index.htm).
It is envisioned that the RCTs will be conducted by multidisciplinary teams of investigators via collaboration between routine medical settings and research institutions. This Notice encourages investigators who believe that they have sufficient expertise and resources to form these collaborative teams to begin considering applying for this RFA.
