LCDB MTA Web Site

To Request Materials from the Laboratory of Cell and Developmental Biology, NIDCR:

Step (1): Fill in the Agreement. Download the Simple Letter Agreement to your computer. Review the instructions on the last page. Fill in the information needed to complete the agreement and save the file.

Step (2): Get an ID#. Send the agreement to the technology transfer office by email. (See the instructions located on the last page of the agreement.)  The agreement will be reviewed and an identification number (ID#) will be assigned.  Please WAIT for the ID# before mailing the agreement.

Step (3): Mail the Hardcopy. Print out two copies and have them signed.  Make sure the MTA ID# is on the bottom, right corner of the first page. We suggest you use certified mail/return receipt or a courier (e.g., FEDEX, UPS, DHL) to mail both copies to the technology transfer office.


Simple Letter Agreement (SLA)
Use the Simple Letter Agreement (SLA) when requesting monoclonal antibodies, polyclonal antibodies, cDNA clones, plasmids, or cell lines. This is the most common transfer agreement used by the Laboratory of Cell and Developmental Biology, NIDCR.
