Disaster Summary for FEMA-1374-DR, Colorado Declaration Date: May 17, 2001 Incident Type: Severe Storms Incident Period: April 11 through April 22, 2001 Counties Declared and Types of Assistance as of May 17, 2001: Baca, Bent, Cheyenne, Crowley, Kiowa, Lincoln, Logan, Morgan, Phillips, Prowers, Sedgwick, Washington, Weld and Yuma for the Public Assistance Program. (14) All counties within the State of Colorado are eligible to apply for assistance under the Hazard Mitigation Grant Program.
