Abstract: (CIT): OPASI ROUNDS Lecture Series Novel Study Designs for Treatment Strategies that Reflect Actual Clinical Practice: Issues They Raise Regarding Implementation and Analysis. Dr. Tsiatis will discuss actual clinical practice; clinicians make a series of treatment decisions over time on a given patient, where each decision depends on the progress of the patient up to that point. For example, decisions on whether to switch treatments, discontinue or augment therapy, or undertake surgical intervention may be made on the basis of patient outcomes such as laboratory measures, adverse events, physical characteristics, and prior treatment history. At key such decision points, there may be several treatment options, and there may be no consensus regarding which option is preferred. Most clinical trials focus on comparing treatment options at only a single point in time, where subsequent decisions on care of the participants are handled in the analysis according to the intention to treat principle. Consequently, such studies do not systematically attempt to identify the best series of options at each of several decision points over time. The Office of Portfolio Analysis and Strategic Initiatives (OPASI) provides the National Institutes of Health (NIH) and its constituent Institutes and Centers (ICs) with the methods and information necessary to manage their large and complex scientific portfolios, identifies - in concert with multiple other inputs - important areas of emerging scientific opportunities or rising public health challenges, and assists in the acceleration of investments in these areas, focusing on those involving multiple ICs.
