ABSTRACT

 Guidelines for Citation of Data

 With the goal of mutual benefit, the National Center for Health Statistics
 (NCHS) requests that recipients of data files cooperate in certain actions
 related to their use.

 Any published material derived from the data should acknowledge NCHS as the
 original source.  The suggested citation to appear at the bottom of all
 tables is as follows:

         Source:  National Center for Health Statistics (1991)

    When cited in a bibliography, the suggested citation should read:

         National Center for Health Statistics (1992).  Public Use File
         Documentation, National Health Interview Survey of Drug and
         Alcohol Use, 1991 (machine readable data file and documentation),
         National Center for Health Statistics, Hyattsville, Maryland.

 The published material should also include a disclaimer that credits any
 analyses, interpretations, or conclusions reached to the author (recipient
 of the data file) and not to NCHS, which is responsible only for the initial
 data.  Consumers who wish to publish a technical description of the data
 should make a reasonable effort to insure that the description is not
 inconsistent with that published by NCHS.

 1. The 1991 NHIS of Drug and Alcohol Use (NHIS-DAU) data were obtained from a
    self-administered questionnaire, given to all special topic sample persons
    18-44 years of age.

 2. The NHIS-DAU Sample person tape consists of:
      a.   The NHIS person record from the core questionnaire (locations 1-
           200)
      b.   Weight fields (locations 201-212).
      c.   The special topic (DAU) data (locations 336-512).

 3. Item non-response was handled differently than in many NHIS special topic
    questionnaires. Because the questionnaire was self-administered and "Don't
    know" was not listed as a valid response, all missing or out of range
    codes were combined into a single "unknown" category (code = 9).

    For most of the data items on this tape, there is a numeric code for each
    respondent. Blanks only appear in the symptom checklists for respondents
    for whom it is definitely known that they never used the drug(lifetime and
    12 month symptoms questions) or that they most recently used it more than
    12 months ago (12-month symptom questions). Respondents for whom lifetime
    use status or recency of use is unknown, have 9's in the symptoms
    checklists when a 'yes' or a 'no' was not given.

    Throughout the data tape, unknowns (9's) include persons who did not know
    the answer to a specific question as well as persons who left large
    sections (or even the remainder of the questionnaire) blank after
    beginning to answer the questions.

    This situation is particularly noteworthy in the symptom checklist where
    the unknowns far exceed the valid codes. Although the unknowns are large,
    their inclusion allows the analyst maximum flexibility in that any
    information that was obtained, is retained.

 4. The response rate for the NHIS-DAU was 75.4 percent. This response rate
    was calculated as follows:

         Household response rate from core (95.7 percent) multiplied by the
         DAU sample person response rate (78.8 percent)

         = 75.4 percent

    The response rate for the NHIS-DAU is somewhat lower than is typical for
    NHIS special topic surveys. This may be attributable, at least in part, to
    the rule that no telephone follow-ups were permitted for persons not at
    home at the time of the initial visit.  Sensitivity of the topic could
    also have contributed to lower response rates.

 5. Comparison of sociodemographic characteristics of respondents and non-
    respondents indicated women were more likely than men to respond to the
    drug questionnaire: 74.3 percent of men and 82.5 percent of women who were
    identified as sample persons eligible for the NHIS of Drug and Alcohol Use
    (NHIS-DAU) responded to all or part of the NHIS-DAU.  Response rates
    varied by race: white men (74.7 percent); black men, 69.9 percent; white
    women, 83.3 percent, black women, 80.3 percent.

 6. Weights:

    Weights must be used to make accurate estimates based on data from the
    National Health Interview Survey. Two weights are included on the 1991
    NHIS-DAU Sample Person tape:

    a.   The Final Basic Weight (location 207-212), calculated for each sample
         person, is the weight that will be used in most analyses of the DAU
         data. This weight is the functional equivalent of the Annual Final
         Basic Weight found on the NHIS Person Record of the Basic Health and
         Demographic component of the survey (i.e the Core questionnaire).

         The DAU Final Basic Weight differs in two ways from that calculated
         for the Core data tape: (1) the DAU sample was re-weighted to account
         for non-response and selection of only one adult per family and, (2)
         the weight field on the core data tape is in binary format and the
         weight field on this tape is in character format.

    b.   The Interim Basic Weight (before age-sex-race adjustment), required
         by some software packages for variance estimation for surveys with
         complex sample designs, is also included on the tape (loc. 201-206).

 7. Estimating numbers of events or conditions

    a.   To reduce respondent error, the recall period for questions about
         some events is limited to two weeks.  These events are:  bed days
         and other restricted activity days, work loss and school loss days,
         and doctor visits.  The two-week variables are found in locations
         98-107 and 120-121.  Estimates of the total number of occurrences
         of these events in the population can be derived as follows:

              Number of events x 26 (number of two-week periods in a year) x
              Final Basic Weight

              =    Total number of events occurring in the population during
                   the data year, i.e. 1991.

         Example: Number of bed days (loc. 100-101) x 26 x Final Basic Weight
         (loc. 207-212) = total number of bed days reported for the population
         in 1991.

    b.   The recall period for acute incidence conditions is also two weeks
         and an annual estimate of the total number of acute incidence
         conditions is calculated using the same procedures as for two-week
         events:

              Number of acute incidence conditions x 26 x Final Basic Weight

              =    Total number of acute incidence conditions occurring in the
                   population during 1991.

              Note: An acute incidence condition is an acute condition with
              onset during the two weeks preceding the date of interview.

    c.   The recall period for information on hospitalizations is 12 months.
         However, in calculating number of discharges and number of days in
         hospital (locations 132-141), only discharges occurring in the past 6
         months are counted.  Therefore, the weighted estimates for these
         events must be calculated as follows:

         (1)  Number of discharges x 2 (number of 6-month periods in a year) x
              Final Basic Weight

              =    Total number of discharges occurring in the population in
                   one year.

         (2)  Number of days in hospital associated with discharges occurring
              in the past 6 months x 2 (number of 6-month periods in a year) x
              Final Basic Weight

              =    Total number of days of hospitalization occurring in the
                   population in one year.

 8. Calculation of rates for events and conditions:

    The number of events or conditions estimated for the population, as
    described in item 7, above, can be used as the basis for calculating rates
    of occurrence of these events (or conditions) per person and per 100
    persons for the total U.S. population and for various population subgroups.

    Note: Only rates can be estimated from these data. The percent of the
    population experiencing a particular type of event during the data year
    cannot be estimated. {The percent of the population experiencing the event
    in the reporting period (i.e. two weeks or 6 months) can be estimated but
    is generally not meaningful.}

 9. Data on hospital episodes and days, based on a 12-month recall are
    locations 122-131. The Final Basic Weight is used for calculating
    estimates of these events in the same way it is used for all other
    person-based variables. These variables do permit estimating the percent
    of the population experiencing a hospital episode in the past year and the
    percent of the population having a specified number of hospital days.

 Data Use Agreement

 In using this documentation and/or the dataset, the recipient of these
 microdata has agreed to the agreement below.  All users of the data are

 Center for Health Statistics.

       DATA USE AGREEMENT - The Public Health Service Act (42 U.S.C. 242m(d))
 provides that the data collected by the National Center for Health Statistics
 (NCHS) may be used only for the purpose for which they were obtained; any
 effort to determine the identity of any reported cases, or to use the informa-
 tion for any purpose other than for health statistical reporting and analysis,
 would violate this statutory restriction and the conditions of the data use
 agreement.  NCHS does all it can to assure that the identity of data subjects
 cannot be disclosed; all direct identifiers, as well as characteristics that
 might lead to identifications, are omitted from the data set.  Nevertheless,
 it may be possible in rare instances, through complex analysis, and with
 outside information to ascertain from the data sets the identity of particular
 persons or establishments.  Considerable harm could ensue if this were done.

       Therefore, the undersigned gives the following assurances with respect to
 all NCHS data sets:

     I will not use nor permit others to use the data in these sets in anyway
     except for statistical reporting and analysis;
     I will not release nor permit others to release the data sets or any part
     of them to any person who is not a member of this organization, except with
     the approval of NCHS;
     I will not attempt to link nor permit others to attempt to link the data
     set with individually identifiable records from any other NCHS or non-NCHS
     data set.
     I will not attempt to use the data sets to learn the identity of any person
     or establishment included in any set; and
     If the identity of any person or establishment should be discovered
     inadvertently, then (a) no use will be made of this knowledge, (b) the
     Director of NCHS will be advised of the incident, (c) the information that
     would identify an individual or establishment will be safeguarded or
     destroyed as requested by NCHS, and (d) no one else will be informed of the
     discovered identity.

       My signature indicates my agreement to comply with the above-stated
 statutorily-based requirements with the knowledge that deliberately making a
 false statement in any matter within the jurisdiction of any department or
 agency of the Federal Government violates 18 U.S.C. 1001 and is punishable by a
