                   1994 NATIONAL HEALTH INTERVIEW SURVEY
                  Disability Phase I Public Use Data File


      Background

      In the United States there are an estimated 35-43 million people
      with physical and mental disabilities.  The Americans with
      Disabilities Act (ADA), which was signed into law in July, 1990,
      is one of the catalysts prompting legal and policy reforms in the
      area of disability.  However, policy-relevant data on disability
      needed to understand its many aspects and impacts is either very
      limited or nonexistent, particularly on a national level.

      In an effort to meet some of these data needs, four federal
      offices (Office of the Assistant Secretary for Planning and
      Evaluation, Health and Human Services; Office of Supplemental
      Security Income, Social Security Administration (SSA); Office of
      Disability, SSA; Bureau of Maternal and Child Health, Health
      Resources Administration) planned several national surveys about
      various aspects of disability in the early 1990's.

      Since many of their interests overlapped, these agencies decided
      to merge their efforts into developing one survey to be included
      with the National Health Interview Survey (NHIS)for two
      consecutive years.  As plans for this survey evolved, other
      organizations with an interest in disability participated
      including:

      Survey Goals and Objectives

      One important goal of the NHIS Disability Survey (NHIS-D) was to
      develop a series of questionnaires that would provide a useful
      set of measures while maintaining a balance between the social,
      administrative, and medical considerations involved in disability
      measurement.  The NHIS-D is not limited to one definition of
      disability; therefore, it will allow analysts from varying
      programs to combine data items in different ways to meet specific
      agency or program needs.  It is designed to collect data that can
      be used to understand disability, to develop public health
      policy, to produce simple prevalence estimates of selected health
      conditions, and to provide descriptive baseline statistics on the
      effects of disabilities.

      Questionnaire Development and Data Collection Methods

      Given such broad objectives for the NHIS-D, it was apparent very
      early in the planning process that it would be impossible to
      collect all of the data needed about disability in one interview.
      Thus, it was decided to use a two phase data collection plan with
      a series of disability questionnaires.

      Development of these questionnaires involved extensive input from
      federal agencies as well as consultants from the research
      community.  Drafts of the questionnaires were reviewed by an
      outside panel of experts from the academic and private research
      community.  Questionnaires were also tested extensively in the
      NCHS Questionnaire Design Research Laboratory and in field tests.

      NHIS-D Phase I:   The Phase I Disability questionnaire was
      administered at the same time as the NHIS Core, and collected
      information about all members of the NHIS households.  For more
      information about the NHIS Core, please refer to the two
      publications cited at the end of this document (1,2).  As with
      the NHIS Core, the NHIS-D Phase I questions were answered by any
      available adult in the household who was knowledgeable about the
      health of other household members.  The Phase I questionnaire
      collected basic data on disability and was used as a screening
      device to determine eligibility for the second phase of the
      survey.

      Although the NHIS-D Phase I questionnaire collected information
      on persons of all ages, for children, a broader perspective on
      disability was taken.  In addition to impairments, information
      was also collected on children with chronic illness or special
      health needs. In order to examine these issues in detail, the
      Phase I questionnaire had 3 sections exclusively for children: a
      section on special health needs of children, a section on special
      education services for children, and a section on early childhood
      development for children under 5 years of age.

      Because of the low frequency of occurrence of disabilities within
      the noninstitutionalized civilian population, Phase I of the
      NHIS-D was fielded over a two year period from 1994 through 1995;
      only the first year of Phase I data is included here.  The sample
      size in 1994 for Phase I was 107,469 persons.

      NHIS-D Phase II:   Eligibility for the second phase of the NHIS-D
      (termed the "Disability Followback Survey or DFS") was based not
      only on responses to the Phase I questionnaire, but also on
      responses to other parts of the NHIS on activity limitation and
      receipt of disability benefits.  The specific inclusion criteria
      for Phase II are summarized in the table at the end of this
      document. These sample selection criteria were applied to the
      unedited data from Phase I.  Interviewing for Phase II began in
      August 1994 and should be completed in 1997.  The data from the
      DFS will be released separately from Phase I. Therefore only a
      brief description of this second phase is included here.

      There are 4 DFS questionnaires: one for children, one for adults,
      one for elderly persons (69 years of age and older) without any
      indication of disability (also called the Supplement on Aging or
      SOA), and one for persons with a past history of polio.

      For children in Phase II, additional information was collected on
      utilization and need for services, functional assessment,
      including emotional and behavioral development, and the impact of
      the child's disability on the family.  The respondent for this
      component was the parent or the adult in the household who knew
      the most about the selected child's health.

      For adults, the Phase II questionnaire obtained more extensive
      information about the persons with disabilities on issues such as
      employment, use of services and benefits, transportation and
      personal assistance needs, housing characteristics, environmental
      barriers, and participation in social activities.  The respondent
      for the Phase II Adult questionnaire was the individual defined
      from the Phase I interview whenever possible.

      Since disabilities increase with age, detailed information on
      older persons, even those currently not experiencing any type of
      disability, is needed for planning purposes.  The SOA
      questionnaire included a number items identical or similar to
      those on the Adult DFS questionnaire since many of the topics are
      very relevant to the elderly population.  In most cases, these
      older adults responded for themselves.

      The Polio Questionnaire collected additional information about
      the symptoms and impact of the illness at the time of initial
      diagnosis of polio and at various times after the diagnosis.
      Information on current health problems was also obtained.
      Whenever possible, self response by the polio survivor was
      required for this component.

      Of the 107,469 persons covered in the 1994 NHIS-D Phase I,
      approximately 28,000 persons were identified for follow up in
      Phase II.

      Content of the 1994 NHIS-D Phase I data files

      Information collected with the NHIS-D Phase I questionnaire has
      been organized into two data files.

      1. Disability Phase I Person File* - this file contains
      information about each person in the NHIS-D Phase I sample.
      Topics covered in this file include:

           Sensory, Communication, and Mobility
             Vision, hearing, mobility aids - all ages
             Communication, understanding, learning - age 5+
             Dizziness, balance, ringing in ears, sense of
                smell + taste - age 18+
           Developmental Disability Conditions(includes polio)-all ages
           Disability
             Activities of daily living (ADL) - age 5+
             Instrumental activities of daily living (IADL) - age 18+
           Functional limitation - age 18+
           Mental health - age 18+
           Services and benefits - age 18+
           Special health needs of children - primarily age under 18
           Early child development - questions vary by specific age
           under 5
           Education - two sets of questions for those age 3-17 and those
           under 3
           Relationship to respondent - age under 18
           Perceived disability - all ages

      2. Disability Phase I Condition File - This file contains
      detailed information on each condition reported as being related
      to certain disabilities.  There are 29 places in the NHIS-D Phase
      I questionnaire that ask for the name of the main condition
      causing that health problem, impairment, limitation, or
      disability.  Each of these 29 items is considered a "source",
      that is, a source from which a condition name emanated.  In most

      cases, only one attributable condition be accepted.  The
      reported condition could be one that was mentioned in the NHIS
      Core or it could be first mentioned in the NHIS-D.  Information
      about each condition mentioned, regardless of its source, is
      contained in this file.  The file structure is similar in format
      to the NHIS Core Condition File.

      The structure of these two files is given below.

      DFS surveys they were eligible for.

      This file contains detailed data for each condition from the NHIS
      Core or NHIS-D Phase I that appears to be related to one or more
      reported disability sources.  It also contains information from
      Core condition records which were reported as causing limitation
      of activity in Core and were not specifically identified as a
      cause of disability with the NHIS-D Phase I questionnaire. When
      the reporting of a disability also required the reporting of a
      causal condition AND one condition could be identified as a
      likely cause, a subset of the data from the Disability Condition
      File was copied onto the Disability Person File.  ICD coding of
      reported conditions was done following the procedures used for
      coding of conditions reported in the NHIS Core.

      The disability source match fields in locations 444-473, identify
      whether or not a person reported that particular disability,
      whether that condition is related to that disability
      source, and if related, how close the match is (see below for
      more details on matching conditions with disability sources).

      Matching the conditions to the sources on the NHIS-D Phase I:

      The process of matching condition records to the specific
      disability sources reported in the NHIS-D was complex. In the
      NHIS-D Phase I questionnaire when a condition was mentioned as
      causing a health problem, impairment, limitation, or disability
      the following items were noted by the interviewer:

           - the name of the condition

           - the question number from which the condition name was
           generated (i.e., its source)

           - whether the condition was previously mentioned during the
           NHIS Core interview and if so, its Core condition record
           number

           - whether the condition was first mentioned in the NHIS-D
           and if so, the condition letter from Section M in the Phase
           I questionnaire (i.e., the location on the questionnaire
           where detailed information was recorded for that condition).

      Because these items were not always entered completely and/or
      correctly, and because respondents were sometimes unable to
      select just one condition as the main cause for each source, it
      was not always possible to definitively match one condition with
      a specific source.  Therefore, it was necessary to develop a
      hierarchical code to indicate the quality of the match.  For the
      various matching situations, the following coding scheme was
      developed and is summarized in loc 401 on the Disability Phase I
      Condition File.  A similar coding scheme was used for each of the
      source-specific match fields (locations 444-473).

           - "0" indicates that some other condition is a better match
           {Found only in the source-specific match fields, locations
           444-473}

           - "1" indicates a perfect match

           - "2" indicates match is an "extra" volunteered condition

           - "3" indicates match is not perfect but good (may be the
           only existing condition record)

           - "4" indicates no good match for a source but match is
           possible

           - "5" indicates a match for one mental health condition
           source but the ICD code is out of the expected range

           - "6" indicates no disability match; condition causes
           limitation of activity (reported only in Core) {Found in loc
           401 only}

           - "7" {Found only on the Disability Phase I Person File to
           indicate no condition records are present for that person}

      If the match was perfect, one condition was clearly attributed to
      a source.  Any condition with at least 1 source code match of 1
      will have location 401 was coded as 1.  A code of 3 was assigned
      when some information was missing but one condition could be
      identified as the most likely match.  This code was also used if
      the person had more than one source but only one condition
      record.  In cases where multiple conditions were reported for one
      source, an attempt was made to select one condition as the "best"
      match.  Preference for selecting this "best" match was dependent
      on where the condition was first reported and whether it was
      chronic or acute. The "best" match condition was assigned a code
      of 3 and all the extra conditions were assigned a code of 2 for
      that source.  If there was no way to identify one particular
      condition as the "best" match for a source, all such conditions
      were given a code of 4.  This was most likely to happen when
      there were multiple chronic conditions but no direct link to any
      source due to interviewer error, or the respondent insisted on
      giving multiple conditions as the cause of a particular problem
      despite interviewer attempts to elicit a singular cause.

      For conditions attributed to mental health sources, a limited set
      of ICD codes was expected. If all of the interviewer notations
      mentioned above were complete for a particular condition but the
      ICD code was outside the expected range, the source match code
      was assigned a 5.  When multiple condition records existed for a
      specific source, the same procedure was used for mental health
      sources as for all other sources as described above.  An attempt
      was made to identify the best match having an expected mental
      health related ICD code which was assigned a 3; all other
      conditions were assigned code 2.

      The final match problem occurred when persons had one or more
      disability sources but no condition records were generated either
      in the core interview or the NHIS-D. Some of these cases may have
      been data entry errors but most are interviews which were
      terminated before detailed condition information was collected
      near the end of the Phase I interview (section M). The code of 7
      was reserved for this situation and appears only on the
      Disability Phase I File.

      Location 401 on the Disability Condition File reflects the code
      for the best match for a given condition record.  When a specific
      source match field was coded as a 1, 3, or 5, a subset of the
      detailed condition data from the Condition File was also
      transferred to the Disability Phase I Person File.  For many
      analysts, this will make it unnecessary to use the detailed
      Disability Condition File.

      With two minor exceptions, the number of conditions per source in
      the detailed Disability Phase 1 Condition File and the recoded
      Disability Phase 1 Person file are identical for each code.  For
      the source First Condition Mentioned Which Causes Trouble With
      Understanding or Communicating (location 446) there are 18 more
      conditions with codes 1 and 3 on the Condition File than the
      Person File.  For Other Disorder (location 459), there is a
      difference of one for code 3.  These differences are due to
      incomplete information.

      Recodes

      There are a limited number of recodes in these files; usually
      they were added to summarize data in several locations.
      Generally, the recodes are located close to the variables upon
      which it was based.  When a recode was related to several non-
      contiguous vases, the recode was placed at the end of the
      relevant section of the data file.

      Response rate

      The overall response for the 1994 NHIS-D was 87.0%.  This
      response rate was calculated as follows:

           Household response rate from core (94.1%) multiplied by
           92.5% who responded to the NHIS-D yielding an overall
           response rate of 87.0%.

      Weights

      Because the NHIS uses a multistage sample design to represent the
      civilian non-institutionalized population of the United States,
      weights must be used to make accurate estimates from the NHIS
      data.  For the NHIS-D, a revised set of weights were calculated
      to adjust for the additional non-response to the NHIS-D. The re-
      weighting process was similar to the non-response adjustment for
      the NHIS Core data and used the age-sex-race categories.  The
      health data from the Core was examined before re-weighting to
      look for any evidence of bias due to differences in health status
      between NHIS-D respondents and non-respondents; no significant
      differences between the two groups were found.  Because of this
      re-weighting, the weights on the Phase I data files are different
      than the corresponding weights on the Core data files.

      There are three fields related to weighting on the Phase I files:

      The Final Annual Basic Weight (location 207-212) will be used in
      most analyses of the Disability Phase I Person File. This weight
      should be used with the Disability data found in locations 401-
      1660 as well as the person level variables in the beginning of
      the file.

      The Interim Basic Weight (before age-sex-race adjustment)
      (location 201-206) is required by some software packages for
      variance estimation for surveys with complex sample designs.
      There are a number of computer programs that yield variance
      estimates for data based on complex sample surveys.   Some are
      based on replication approaches and others are based on Taylor
      linearization approaches.  In addition to the Interim Basic
      Weight, the Phase I files also include the Full Sample Stratum
      Identifier (location. 188-190), the Pseudo Primary Sampling Unit
      (PSU) Codes (Location. 198-200), and the Type of PSU (Location.
      193) to provide the analyst the capability of using such variance
      estimation procedures.  These variables and weights are necessary
      for directly calculating sampling variances.

      The last weight field, the Chronic Condition Prevalence and
      Incidence Factor (location 213-223 with an implied decimal) must
      be used if analysts wish to match the Disability Phase I Person
      File to the NHIS Core Condition File to make prevalence estimates
      for the NHIS-D sample of the specific chronic conditions included
      on one of the six condition lists in the Core questionnaire.
      Again, this Factor field in the Disability file has been re-
      weighted and differs from the same weight field on the NHIS
      core.  See below for more information on prevalence estimation
      with the NHIS-D Phase I data.

      Estimating annual numbers of events:

      To reduce respondent error, the recall period for questions about
      some events is limited to two weeks.  These events are bed days,
      work loss and school loss days, and doctor visits.  The two week
      variables are found in locations 98-107 and 120-121.  Estimates
      of the total number of occurrences of these events in the
      population can be derived as follows:

                Number of events x 26 (number of two week periods in a
                year) x Final Annual Basic Weight

                = Total number of events occurring in the population
                during 1994.

           Example: Number of bed days (Location. 100-101) x 26 x Final
           Annual Basic Weight (Location. 207-212) = total number of
           bed days reported for the population in 1994.

      The recall period for information on hospitalizations is 12
      months.  However, in calculating the number of discharges
      (Locations 132-133, 137-138), only discharges occurring in the
      past six months are counted.  Therefore, the weighted estimates
      must be calculated as follows:

                Number of discharges x 2 x Final Annual Basic Weight

                = Total number of discharges occurring in the
                population in 1994

      Limitation in estimating prevalence of conditions

      National prevalence estimates of conditions can be made in two
      ways using the Disability Phase I Person File.  Prevalence can be
      estimated directly for the specific conditions listed in Section
      B of the questionnaire (such as cerebral palsy and spina bifida)
      using the Final Annual Basic Weight.  Prevalence estimates can
      also be made for the conditions found on any of the 6 condition
      lists in the Core questionnaire.  This would involve linking the
      Phase I Person File to the Core Condition file and, as mentioned
      above, the NHIS-D Chronic Condition Prevalence and Incidence
      Factor would have to be used to make these estimates. It should
      be noted that the Disability Phase I Condition File is NOT
      intended to make national prevalence estimates of conditions
      contained in that file.  The purpose of this file is to provide
      more detail on reported conditions for which there is some
      specific or possible relevance to a source of disability.

      Item non-response

      Two different types of item non-response are possible in the data
      files: responses of "don't know" or "refused" were assigned codes
      of "9", "99", or "999"; when a question was not answered when it
      should have been or the answer given was not possible, "Not
      ascertained" was assigned (a code of "8", "98", or "998").

      Linkable files

      In addition to the files mentioned above, the following NHIS data
      files exist for 1994.

      For all the NHIS-D persons:
           Access to health care
           Health Insurance
           Imputed Family Resources(scheduled for release in late 1996)

      For a selected sample adult in each family:
           Year 2000 Objectives (including environmental health,
           tobacco, occupational safety and health, heart disease and
           stroke, clinical and preventive services)
           AIDS Knowledge and Attitudes

      If an analyst matches the Disability Phase I data to the NHIS
      Core files or other files with information on the entire NHIS-D
      Phase I sample, the weights on the Disability files should be
      used.  However, if the analyst is matching to the sample adult
      files listed above, the correct weight to use is the sample
      person weight found on the Year 2000 or the AIDS file.

      Guidelines for Citation of Data

      With the goal of mutual benefit, the National Center for Health
      Statistics (NCHS) requests that recipients of data files
      cooperate in certain actions related to their use.  Any
      published material derived from the data should acknowledge NCHS
      as the original source.  The suggested citation to appear at the
      bottom of all tables is as follows:

      When cited in a bibliography, the citation should read:
           National Center for Health Statistics (1996).  Data File
           Documentation, National Health Interview Survey of
           Disability, Phase 1, 1994 (machine readable data file and
           documentation), National Center for Health Statistics,
           Hyattsville, Maryland.

      The published material should also include a disclaimer that
      credits any analyses, interpretations, or conclusions reached to
      the author (recipient of the data file) and not to NCHS, which is
      responsible only for the initial data.  Consumers who wish to
      publish a technical description of the data should make an effort
      to insure that the description is not inconsistent with that
      published by NCHS.

        DIAGNOSTIC RECODE C

         Chronic Conditions and Impairments

        I.     Purpose - Recode C consists of 134 3-digit categories
               of chronic conditions and impairments.  The categories
               correspond to the conditions  that appear on the six (6)
               NHIS check-lists of selected chronic conditions.  These
               categories are used by NHIS to produce prevalence
               estimates of selected chronic conditions.

        II.    Format and Content - The first digit of this 3-digit
               recode identifies  the specific chronic condition
               check-list number, i.e., 1-6, that corresponds to the
               number of the list used on the NHIS questionnaire.
               The second and third digits are numbered sequentially
               within each of the  6 lists.  Each condition group
               contains between 15 and 43 2-digit categories.

       III.    Definition of Chronic Condition and Impairment - In most
               cases, a chronic condition or an impairment is any
               departure from normal health with onset more than 3
               months from date of interview.  Certain chronic
               conditions and impairments are classified as chronic,
               however, regardless of date of onset.  Complications of
               pregnancy are always classified as acute.  All chronic
               conditions and impairments are counted  whether or not they
               necessitated activity restriction or medical care.
               Impairments coded with an "X" in the first digit are, by
               definition, chronic.

       This recode is different from Chronic Condition Recode 3, used prior
