Form Description
This form is to be used by creditors in order to facilitate the federal government's compliance with commercial garnishment orders as mandated by section 9 of the Hatch Act Reform Amendments of 1993, Public Law 103-94. This is a voluntary form. However, by providing information in a uniform manner, the form simplifies the actions which must be taken by federal agencies in order to comply with commercial garnishment orders.
