Register for LBPH Services

It's easy to start receiving books and other materials from LBPH:

1. Complete an application. You can view and print an LBPH application using the free Adobe Acrobat Reader or contact us to mail you an application packet.
2. Have your application certified. We will need the signature of a doctor, nurse, teacher, caseworker, or other competent authority to verify your eligibility before we process your application.
3. Mail the application to:

That's it! Your LBPH library services will start as soon as your application is processed, usually within two business days. You can tailor the services to your needs at any time.
