La santé des personnes agées, une priorité pour le NIEHS
Faisant partie des organismes engagés dans le programme Aging Initiative de l'Agence américaine pour la protection de l'environnement, le NIEHS (Institut national américain des sciences de l'hygiène du milieu) considère la recherche sur le vieillissement comme une priorité importante. Ce mois-ci, la rubrique NIEHS News (p. A756)met l’accent sur les études relatives au vieillissement actuellement menées et parrainées par l’institut dans les domaines de la neurotoxicologie, du neurodéveloppement, des maladies respiratoires, du cancer et d’autres maladies chroniques, ainsi que dans le domaine des maladies héréditaires, leurs liens avec le vieillissement.
L’âge influe sur l’hygiène du milieu
Les enfants commencent, à juste titre, à faire l’objet d’une grande attention dans le cadre de l’hygiène du milieu, mais qu’en est-il des problèmes d’hygiène du milieu spécifiques auxquels leurs grands-parents sont confrontés&npsp;? La rubrique Focus (p. A760) envisage l’avenir de la recherche sur l’hygiène du milieu à travers des études sur la manière dont le monde dans lequel nous vivons contribue au vieillissement et, à l’inverse, sur la façon dont les adultes vieillissants influencent leur environnement.
L’Europe veut réformer la réglementation des produits chimiques
La rubrique Sphères d’influence (p. A766) aborde le sujet concernant la proposition de loi controversée connue sous le nom de REACH (enregistrment, évaluation et autorisation des substances chimiques), qui doit être présentée devant le Parlement européen à la fin de l’année. S’il est appliqué, le projet REACH va modifier profondément la réglementation des produits chimiques industriels en Europe, en touchant non seulement les pays et les industries qui commercialisent ces substances, mais peut-être aussi en suscitant, de la part d’autres nations, des changements dans leurs politiques de réglementation de celles-ci.
Plein feux sur le dépistage du mélanome
Le mélanome fait partie des cancers provoquant le plus de décès, mais le dépister avant qu’il ne soit trop tard pour le traiter peut se révéler délicat. La rubrique Innovations (p. A770) fait le point sur les avancées en dermoscopie : cette technique repose sur l’utilisation d’un instrument manuel pourvu d’un bon éclairage et d’une loupe grossissante, lequel permet aux médecins de mieux examiner la peau afin d’y repérer toute tache suspecte.- L'introduction récente des mathématiques appliquées a abouti à l'apparition d'instruments qui permettent également de déterminer si des taches suspectes sont potentiellement cancéreuses.

Xénobiotique et cellules de l’anémie de Fanconi
De nombreux éléments démontrent la sensibilité des cellules de l’anémie de Fanconi (AF) aux xénobiotiques à activité oxydoréductrice. Les mécanismes de la toxicité semblent expliquer l'incapacité des cellules de l'AF à supporter le stress oxydatif.a Pagano et coll. (p. 1699) commentent une étude récente qui a révélé une sensibilité excessive des cellules du groupe de complémentation A de l'AF à la toxicité du chrome VI et postulé que l’absence d’élimination des liaisons croisées Cr-ADN et la formation de liaisons croisées impliquant le Cr(VI) peuvent être les deux constituants du mécanisme de la cytotoxicité due au Cr(VI)
Lymphome non hodgkinien et mesures préventives en Suède
Une comparaison entre les périodes 1971-1990 et 1991-2000 a révélé un ralentissement dans plusieurs pays de l'incidence annuelle, normalisée par tranches d’âge de la population, du lymphone non hodgkinien (LNH). L'immunosuppression et les pesticides tels que l'acide phénoxyacétique, les chlorophénols, les chlordanes et les dioxines ont été associés au LNH. Hardell et Eriksson (p. 1704) envisagent la possibilité que ces statistiques du cancer reflètent l’impact des réglementations instaurées dans les années 70, et la baisse consécutive de l’exposition de la population à ces produits chimiques.
Tremblement essentiel et plombémie
Le tremblement essentiel (TE) est un maladie neurologique répandue impliquant le cervellet, et le plomb est connu pour être la cause de dommages cérébelleux. Louis et coll. (p. 1707) ont analysé l’historique de la vie professionnelle d’individus et la plombémie dans une étude de TE équilibrée par la fréquence. Bien que l'exposition au plomb sur le lieu de travail ait été identique chez les patients atteints de TE et les sujets du groupe témoin, une corrélation a été établie entre plombémie et TE. Cette corrélation est peut-être due à une exposition accrue au plomb ou à une cinétique différente de cet élément chez les patients souffrant de TE.
Evolution de la mortalité causée par la chaleur aux Etats-Unis
L’élévation de la température et du taux d’humidité laissent présager un accroissement, à long terme, de la mortalité liée à la chaleur. Davis et coll. (p. 1712) ont calculé le surcroît annuel de mortalité lié jours où les températures apparentes ont dépassé une valeur seuil dans 28 agglomérations des Etats-Unis de 1964 à 1998. Le taux de mortalité causé par la chaleur a diminué de manière significative dans 19 de ces 28 villes. Cette désensibilisation au fil du temps peut être attribuée aux adaptations technologiques, infrastructurelles et biophysiques ainsi qu'à l'avènement de la climatisation.
Exposition au phtalate chez les femmes enceintes
Certains phtalates perturbent la fonction endocrine et provoquent une toxicité au niveau de la reproduction et du développement. Adibi et coll. (p. 1719) ont mesuré les taux de phtalates contenus dans des échantillons d'air individuels prélevés chez des cohortes de femmes enceintes à New York ainsi qu'à Cracovie, en Pologne. Des prélèvements d'urine ont également été effectués chez des femmes new-yorkaises. Les résultats font état d'une exposition considérable aux phtalates par inhalation au cours de la grossesse et révèlent des corrélations non négligeables entre les taux de phtalates dans l'air et dans les urines.
Présence PBDE dans le lait humain
L'utilisation d'agents ignifuges à base de diphényléther polybromé (PBDE) est autorisée aux Etats-Unis, mais interdite dans certains pays d'Europe. Schecter et coll. (p. 1723) ont analysé des échantillons de lait de mères allaitantes, âgées entre 20 et 41 ans, qu’ils ont prélevés dans une banque de lait et dans une clinique pour femmes au Texas. Les taux de PBDE dans le lait des femmes texanes sont semblables aux taux relevés dans le sang et les tissus adipeux des femmes de Californie et de l'Indiana et sont 10 à 100 fois supérieurs aux taux constatés dans les tissus humains en Europe. Transporteur de choline chez l’embryon d’oursin
Le transporteur de choline à haute affinité régule la synthèse d'acétylcholine et est affecté par des agents neurotoxiques dans le cerveau de mammifères. Qiao et coll. (p. 1730) ont évalué le développement et la réponse du transporteur de choline chez l'oursin. Les résultats montrent que les oursins en phase de croissance possèdent un transporteur analogue à celui des mammifères et, comme chez ces derniers, il y joue un rôle fondamental dans l’activité d'acétylcholine. Le modèle de l'oursin peut donc se révéler utile pour le criblage robotisé à haut débit d'agents neurotoxiques de développement.
Exposition au chlorpyriphos au cours du développement
Aldridge et coll. (p. 1736) ont déterminé si la neurotoxicité de développement du chlorpyriphos (CPF) avait des effets sur la signalisation de la sérotonine (5HT). Des rats ont été exposés à du CFP au cours de quatre phases différentes de développement. Les résultats montrent que le CPF affecte pendant une période brève de la grossesse les récepteurs de 5HT, le transporteur de 5HT présynaptique et la transduction des signaux transmis par la 5HT. Les effets contribuent vraisemblablement à l’aspect non cholinergique de la neurotoxicité de développement du CPF.
L’arsenic accélère l’athérogénèse chez les souris déficientes en apolipoprotéine E-/-
Des études effectuées sur des humains montrent une corrélation positive entre l’arsenic présent dans l’eau potable et le risque de maladies vasculaires. En utilisant une souris transgénique déficiente en apolipoprotéine, modèle animal de l’athérosclérose humaine, Simeonova et coll. (p. 1744) démontrent une augmentation des lésions aortiques chez les souris transgéniques traitées à l’arsenic comparés aux souris transgéniques non traitées. Aucune lésion semblable n’a été observée chez la souris de type sauvage. Des études in vitro menées en parallèle laissent supposer que l’activité inflammatoire endothéliale joue peut-être aussi un rôle dans les effets vasculaires liés à l’arsenic.

Séminaire consacré au déficit en –1 antitrypsine
Le déficit en –1 antitrypsine (AAT) est un trouble génétique héréditaire qui peut conduire à la détérioration de la fonction pulmonaire chez les adultes et provoquer des maladies hépatiques chez les enfants. Sharp et coll. (p. 1749) présentent un aperçu d’un séminaire consacré au déficit en AAT et visant à a) étudier les facteurs de morbidité et de mortalité, b) à définir les besoins de la recherche, et c) à créer des opportunités de collaboration.
Exposition à de la vermiculite contenant de l’amiante à Libby (Montana)
De la vermiculite contenant de l’amiante a entraîné une contamination à grande échelle à Libby dans le Montana. Un programme d’analyses médicales a été lancé afin d’étudier les corrélations entre les anomalies radiographiques pleurales et l’exposition des participants à la vermiculite. Peipins et coll. (p. 1753) ont déterminé que les facteurs les plus couramment associés aux anomalies pleurales sont les suivants&npsp;: avoir été employé chez W.R Grace, être âgé, avoir vécu sous le même toit qu’un employé de W.R. Grace et être de sexe masculin. Les expositions environnementales et autres facteurs de risques non professionnels pouvaient également être à l’origine d’anomalies dues à l’amiante.
Biomarqueurs urinaires d’exposition au carburéacteur
Le carburéacteur (JP-8) peut être une source importante d’exposition au benzène, au naphtalène et au 1- et 2-naphtols. Serdar et coll. (p. 1760) ont mesuré le taux de ces composés dans des prélèvements d’urine effectués chez des employés classés selon le niveau d’exposition au JP-8&npsp;: faible, moyen et élevé&npsp;; ils ont identifié de fortes corrélations entre les taux après exposition de biomarqueurs à base de naphtalène dans les urines et les taux de naphtalène dans l’air et dans l’haleine. Les naphtols constituent probablement le marqueur le plus utile, compte tenu de leur grande abondance et de la lenteur avec laquelle ils sont éliminés.
Travail à la ferme et performances neurocomportementales
Kamel et coll. (p. 1765) ont recueilli des informations relatives au travail à la ferme au cours d'un entretien structuré et ont évalué les performances neurocomportementales à l'aide d'une série de tests. Les analyses ont été corrigées par des facteurs parasites, dont l’âge, le sexe, le niveau d’étude et l’acculturation. Plus longue était la durée du travail à la ferme, moins bonnes se révélaient les performances. Des déficits liés à la durée du travail à la ferme ont été constatés aussi bien chez les anciens agriculteurs que chez les personnes encore en activité. La baisse des performances était liée à une exposition chronique, même en l'absence d'empoisonnement par les pesticides.
La santé infantile
Pollution de l’air ambiant et conséquences néfastes sur les grossesses
Liu et coll. (p. 1773) ont étudié le lien entre la naissance avant terme, la faiblesse du poids à la naissance et le retard de croissance intra-utérine parmi des naissances vivantes et les taux ambiants de dioxyde de souffre et d'azote, de monoxyde de carbone et d'ozone à Vancouver, au Canada, entre 1985 et 1998. Les résultats montrent que l'exposition à des concentrations relativement faibles de polluants gazeux dans l'air au cours de différentes périodes de la grossesse sont associées à des effets néfastes sur l'issue de cette dernière.
Mesure des pesticides dans le liquide amniotique
Des études sur les pesticides présents dans l'urine de femmes enceintes et dans le méconium laissent supposer que le fœtus est exposé aux pesticides. Cependant, le prélèvement de liquide amniotique lors d'une amniocentèse est le seul moyen de caractériser l'exposition du fœtus en début de grossesse. Bradman et coll. (p. 1779) ont mesuré non seulement le taux de pesticides organophosphorés, mais aussi celui des pesticides au carbamate ainsi que leurs métabolites et les métabolites de pyréthrinoïde, les herbicides et les composés phénoliques chlorés contenus dans des échantillons de liquide amniotique. Bien que relativement bas, les taux de pesticides dans le liquide amniotique révèlent une exposition directe du fœtus à un stade précoce.
Exposition prénatale aux phtalates et durée de la grossesse
Le di-(2-ethylhexyl) phthalate (DEHP), plastifiant courant intervenant dans la composition du polychlorure de vinyle souple, est un polluant omniprésent. Latini et coll. (p. 1783) ont mesuré la durée de la grossesse ainsi que le DEHP et/ou son métabolite principal, le mono-ethylhexyl phthalate (MEHP) dans le sang du cordon ombilical de 84 nouveaux-nés consécutifs. Du DEHP et/ou du MEHP a été trouvé dans 88,1 % des échantillons. Les enfants exposés au MEHP étaient nés à un stade beaucoup plus précoce que ceux qui n'y avaient pas été exposés (p = 0,033).

Questions éthiques concernant la recherche sur l'hygiène du milieu
La recherche sur l'hygiène du milieu recouvre l'identification des risques, l'évaluation des mécanismes biologiques, l'instauration de mesures destinées à atténuer les dommages et l'identification des populations potentiellement exposées. Un certain nombre de défis éthiques, juridiques et politiques sont spécifiquement associés à chacun de ces axes principaux. Sharp et coll. (p. 1786) examinent de nombreuses questions se révélant de plus en plus complexes tandis que génétique et biologie moléculaire sont utilisées pour étudier les diverses influences du milieu sur la santé des hommes et de l'environnement.
