Do you have Purchase Request Numbers?

Please email dpearson@ncifcrf.gov to secure a block of Purchase Request numbers for your Program Area. A Purchase Request number is required in the upper right hand corner of the form before sending to the Purchasing Department.

PLEASE NOTE: If you receive an error message during calculations - those using Microsoft Word 2003 will need to change their Security setting to Low or Medium (go to Tools/Options/Security/Macro Security to change these settings); those using prior versions incurring the same problem can change their security settings; however, you may continue to experience problems and may need to upgrade Microsoft Office 2003 in order for the calculating macros to function properly.
