Physical Exam 04/03/XX HEENT: Small, movable nodule at left inferior pole of thyroid adjacent to isthmus. Rest of lobe normal. Remainder of exam normal.

Operative Findings

No involved nodes on either side.
   
Pathological Reports

04/14/XX

Papillary CA of thyroid completely removed at surgery.

   
Treatment

04/09/XX

Surgery: Left thyroid lobectomy with portion of isthmus. Tumor size 1.5 cm.
  04/10/XX Hormone: Began Synthroid.
  06/01/XX Radiation: I-131 ablation therapy
