Supplemental Instructions for Form PHS 398

(For New, Competing Continuation, and Revised Applications)

Follow the instructions for completing the Form PHS 398 (http://grants.nih.gov/grants/funding/phs398/phs398.html). Applicants must use the currently approved version of the PHS 398. In addition, follow the specific instructions for new, competing continuation, and revised applications provided in the MBRS IMSD Program Announcement (PAR-05-132). Also, use the sample IMSD tables 1-10 (http://www.nigms.nih.gov/funding/imsd_sample_tables.html) as appropriate in preparing the grant application. Applications may be returned without review if these instructions are not followed.

Supplemental Instructions for Form PHS 2590

(For Non-competing Continuation Grant Applications)

Progress reports to continue support of a Public Health Service (PHS) grant must be submitted to NIGMS two months before the beginning date of the next budget period using the most current PHS 2590 (forms and instructions are available at http://grants.nih.gov/grants/forms.htm). In addition to the PHS 2590 instructions follow the specific guidelines suggested here and use the sample IMSD tables 11-15 (http://www.nigms.nih.gov/funding/imsd_type5.html) as appropriate to prepare the progress report. All progress reports must be submitted to the centralized mailing address:

Submit the completed, signed original progress report and one copy (with required signatures).

Note that this is a change in business process. Progress reports should no longer be submitted directly to the NIH awarding component.

Grantees access a website to determine which progress reports are due. The Office of Policy for Extramural Research Administration, OER, National Institutes of Health (NIH) hosts the website located at: http://era.nih.gov/userreports/pr_due.cfm.

Grantees are responsible for periodically checking the list, which is updated on/around the 30th of each month. In addition to this website, e-mail reminders are sent to the PI.

For grantee institutions and PIs registered in the eRA Commons, the progress report due information is available in the Commons Status system. Commons-registered institutions and PIs also have access to pre-populated face pages for the 2590 Progress Report via Status. Specific Instructions for preparing IMSD grant Progress Report:

Streamlined Non-competing Award Process (SNAP) Instructions for Submitting the Progress Report

Complete Face Page (Form Page 1) except for items 8a and 8b, the Progress Report Summary (Form Page 5) and the Key Personnel Report (Form Page 7). Complete the Budget Form pages 2 and 3 (PHS 2590) only if any of your responses to SNAP are “yes”. Also, complete the Checklist Page (Form Page 6) only if there is a change in performance site(s) that will affect the facilities and administrative costs and/or if program income is anticipated. If program income is anticipated the Checklist should reflect the amount and source(s). The Progress Report should begin on Form Page 5.

The Progress Report should summarize the progress achieved in the reporting period with respect to the IMSD Program goals. The narrative part is limited to three pages, and numerical and other data may be presented in tabular form (see sample IMSD tables 11-15, included at http://www.nigms.nih.gov/funding/imsd_type5.html; tables and figures are not counted in the three-page limit). The full evaluation report and the reprints of IMSD student-authored peer-reviewed papers acknowledging IMSD support should be submitted as Appendix.

Narrative Section (limit to 3 pages):

  • Specify the start/end dates of the one-year period covered by the progress report (i.e. the “reporting period”).
  • Provide a brief summary (2-3 paragraphs) of the overall progress made during the reporting period.
  • Restate the original goals and objectives, and briefly describe the progress in specific relation to these goals and objectives.
  • Describe any significant professional recognition of the IMSD students and participating faculty.
  • Describe how the IMSD Program evaluation results for this reporting period have been used to refine/improve the Program activities.
Tabular Data Section:
  • Use the sample IMSD table formats to submit data in the IMSD Annual Progress Report.
  • Provide IMSD Student and Faculty Participant Data.
  • Provide IMSD Undergraduate and Graduate (as applicable) Student Graduates Data.
  • List all IMSD Program participants on the “Key Personnel Report” Form page 7 (PHS 2590) for the one-year reporting period. Specify the academic status (i.e. year level as grad/undergrad) of the student participants, and percent effort of participating faculty and administrators.
  • Provide complete citations (including title, year, journal, volume, inclusive pages) of peer-reviewed publications, and those in press (omit manuscripts in preparation) with IMSD student co-authors, in which IMSD support is acknowledged, for the one-year reporting period only (see the sample IMSD table for Publications of Research Completed by IMSD Students).

Appendix: Limit the appendix to reprints of published papers and preprints of papers in press resulting from and acknowledging MBRS-IMSD support, and full evaluation reports.

Use the relevant sample IMSD tables to provide the data in a tabular form. Tables 1-10 (http://www.nigms.nih.gov/funding/imsd_sample_tables.html) are suggested for the New (Type 1), Competing Continuation (Type 2), and Revised Applications, while Tables 11-15 (http://www.nigms.nih.gov/funding/imsd_type5.html) are suggested for the IMSD Annual Progress Report (Type 5 application).
