PAYBACK ACTIVITIES CERTIFICATION (PHS 6031-1) NOW AVAILABLE

EFFECTIVE DATE:  The revised "Ruth L. Kirschstein National Research 
Service Award Fellowship Activation Notice" (PHS 416-5, Rev. 6/02), 
"Information and Termination Notice for Fellows and Trainees Completing 
a Ruth L. Kirschstein National Research Service Award" (PHS 416-7, Rev. 
6/02), "Payback Agreement" (PHS 6031, Rev. 6/02) and "Annual Payback 
Activities Certification" (PHS 6031-1) are now available and will be 
accepted immediately.  Use of the previous versions (Rev. 12/98) will 
be accepted through December 31, 2002.  However, the 12/98 versions of 
the 6031 and 6031-1 will continue to be accepted indefinitely. Note, 
there are no changes in the data items of these forms. 

The revised instructions and forms are available at 
http://grants.nih.gov/grants/forms.htm#training.  The forms available 
on the website have been modified to enable the fields to be filled in 
directly using Adobe Acrobat Reader Software.  Free Adobe Software may 
be accessed at http://www.adobe.com/products/acrobat/readstep.html.  If 
you have any questions about using the new forms and the Adobe Acrobat 
Reader Software process, see "INSTRUCTIONS FOR DOWLOADING DOCUMENTS AND 
ELECTRONIC FORMS" at http://grants.nih.gov/grants/edocs.htm.

The forms will also be available in Rich Text Format (RTF), a 
standardized way to encode various text formatting properties, such as 
bold characters and typefaces, as well as document formatting and 
structures.  RTF is supported across a number of platforms.  RTF files 
can be opened in many word processors and other RTF-aware software 
packages with much of its formatting left intact.  In addition, many 
word processing programs will also allow users to save a document in 
RTF when you select "Save As…"

NOTE:  Other software packages for completing these applications may be 
available from other sources; however, it is essential that the type 
size and format specifications are met or the application will be 
returned without review.
