This addendum is to clarify instructions listed under Section IV.6. Other Submission Requirements and Information, Supplementary Instructions in RFA-RM-08-029.

Submitting the T-R01 Research Plan:  The 8 page Research Plan consisting of sections describing the Challenge and Potential Impact (one page), Approach (five pages), Appropriateness of the T-R01 mechanism (one page) and Timeline (one page), should be prepared as a single pdf  prior to uploading as Attachment 5 (Research Design and Methods) into the PHS 398 Research Plan.

All other aspects of this FOA remain unchanged.
