Information to Include in Your Donation Letter to NCI

All letters should be addressed to:

Donations may be made by check or money order payable to the National Cancer Institute.

Donor Information
* All donors are sent an acknowledgement letter.

• Name (contact person)
• Company Name (if applicable)
• Street Address
• City, State and Zip Code
• Phone Number
• E-mail address (optional for follow-up questions from NCI)

Research Designation (optional)
All donations received are applied toward cancer research at NCI as determined by the Director. However, if you want your donation to be used on a specific type of cancer research, you may state it in your letter.

Memorial Donations (optional)
If your donation is a memorial, please include the name of your loved one in your letter. Additionally, please list the name(s) and address(es) of the person(s) whom you would like NCI to acknowledge for the memorial donation. This might include a spouse, sibling, parent, etc.

Honorary Donations (optional)
If your donation is in honor of a person, please write the name of that person in your letter. Additionally, please list the name(s) and address(es) of the person(s) whom you would like NCI to acknowledge for the honorary donation. This might include the honoree, a spouse, a sibling, a parent, etc.
