How To Enter
Please read the following information before completing an entry form.
1. Fill in your contact informationwhere all correspondence will be sent.
2. Fill in your entry informationthe title of entry and organization name as you wish it to appear on a certificate should you win an award. You may submit as many entries as you wish.
3. Click "Finish and Pay" when you are ready to finalize your entries.
Note: only English language sites will be accepted.

Make sure your site is functional during the judging period,
December 15, 2008 - January 16, 2009.
Web sites should support the latest browsers (Firefox, Internet Explorer, etc.)

If needed, please provide a temporary password for judging. Password must be active during the judging period — 12/15/2008 - 1/16/2009.

Entry Fee: $52.00 per entry. If you wish to enter your site into any of our special awards categories, there will be an additional $10.00 charge for each catagory.

Payment Methods: VISA, MasterCard, and checks are accepted. Make checks payable to the Health Information Resource Center or HIRC. (Fed. Tax I.D. # 36-3559293)

Entry Deadline: Friday, December 12, 2008
