There are three different background investigation questionnaires, but you will complete only the one that is appropriate for your job. Current employees may not have to complete these forms if they are already on file. Questionnaires must be completed through the Office of Personnel Management's secure online system, Electronic Questionnaires for Investigations Processing (e-QIP). International applicants may need to complete a printed version of this form by hand and deliver it to Personnel Security (Building 31, Room 1B03).
