The authors define menopause as the permanent cessation of menstruation; perimenopause refers to the time period preceding menopause, when fertility wanes and menstrual cycle irregularity increases. The chapter covers the physiology, symptoms, and appropriate diagnostic tests of perimenopause; menopause and postmenopausal hormone therapy; and the benefits and risks of postmenopausal hormone replacement therapy (HRT). Low-dose oral contraceptives are a therapeutic mainstay in perimenopause, whereas postmenopausal HRT has been a common method of symptom alleviation after menstruation ceases.
