Abstract: This editorial comments on the role of physical fitness in type 2 diabetes. A study of physical activity among men who had type 2 diabetes found a strong relationship between measures of cardiovascular fitness and mortality. Another study found that diabetes control was significantly improved in experimental participants who engaged in physical activity and who developed diabetes during the study. Evidence also suggests the value of an exercise program in the prevention of type 2 diabetes in high risk people. Several studies have demonstrated a reduction in the progression to frank diabetes in high risk people. In addition, findings from several studies suggest that the amount of exercise needed to achieve a cardioprotective benefit is modest. Weight gain during adulthood in genetically susceptible people is one of the most powerful predictors of the development of diabetes. Although caloric intake has not changed over the past two decades in the United States, obesity is increasing. This increase in obesity is most likely the result of a reduction in physical activity. Thus, a comprehensive program of exercise should be an integral part of diabetes care for children, adolescents, and adults. 18 references.
