Chronic Kidney Disease
The National Kidney Disease Education Program, along with the American Society of Nephrology, has developed a slide presentation about improving patient outcomes for chronic kidney disease (CKD) in the primary care setting. The slides feature the latest data from the U.S. Renal Data System Annual Data Report and outline the role primary care professionals can play in detecting and treating CKD.
