Abstract: This chapter on insulin therapy in type 2 diabetes is from a reference book that explores the pharmacokinetics of insulin and insulin programs. The book focuses on the latest blood glucose self-monitoring equipment and assessment strategies that can achieve optimal glycemic control and thus reduce the occurrence of complications including retinopathy (eye disease), neuropathy (nerve disease), nephropathy (kidney disease) and cardiovascular disease. In this chapter, the authors discuss indications for insulin therapy in type 2 diabetes, patients' and physicians' concerns about insulin therapy in type 2 diabetes, the benefits of insulin therapy, combination therapy with oral agents and insulin, intensified insulin regimens, and the role of insulin in type 2 diabetes treatment progression. The authors conclude that the combination of insulin with oral agents will remain an important first step in this form of diabetes, but many patients will progress to basal-prandial insulin regimens similar to those used in type 1 diabetes. 4 tables. 23 references.
