Title: Focal Fatty Liver: More Than Just a Radiographic Curiosity?.

Abstract: This article comments on an accompanying case report of a woman with multifocal nodular nonalcoholic steatohepatitis (NASH), in whom radiologic abnormalities dramatically resolved after she received the drug rosiglitazone. Nonalcoholic fatty liver disease (NAFLD) has been associated with insulin resistance, and insulin sensitizers have been previously shown to improve liver biochemistry and histology in NASH. This patient’s response to rosiglitazone included improvement in glycemic control, as well as improvement in insulin sensitivity in both the fasting and the fed states. In the commentary article, the authors review the different types of focal hepatic steatosis, note the lack of current understanding of focal fatty liver, briefly consider the natural history of focal hepatic steatosis, and discuss the physiology of vascular supply to the liver. The commentary concludes that the case report demonstrates an interesting variant of a very common cause of liver disease among both adults and children in the United States. The case also underscores the importance of liver imaging techniques in distinguishing focal hepatic steatosis from other mass lesions in the liver. 16 references.
