Abstract: Diabetes is a disease characterized by abnormal metabolism of carbohydrates, proteins, and fats. This chapter on the pathophysiology of the metabolic disorder is from a comprehensive text that serves as a resource for all health professionals, community professionals, and individuals who provide education to individuals with diabetes. The authors note that newly discovered hormones and systems that regulate energy balance have increased understanding of normal physiology and the pathophysiology of diabetes. The chapter covers normal fuel metabolism, the role of hormones, the different types of diabetes, and the pathogenesis of type 1 diabetes and of type 2 diabetes. Type 1 diabetes results from autoimmune beta cell destruction, leading to absolute insulin deficiency. Type 2 diabetes is a multihormonal pathophysiology involving a progressive insulin secretory defect along with insulin resistance. This disease progresses from an early asymptomatic state with insulin resistance, to mild postprandial hyperglycemia, to clinical diabetes requiring drug therapy. Obesity, weight gain in adulthood, and physical inactivity are environmental factors affecting the progression at all points along the continuum.
