173.Telephone Service

Telephone Service Request

The Telephone Service Request Form (TSR) is used to request telephone services when no purchase is involved. A number of telephone services can be requested by using the on-line TSR located at http://tsr.cit.nih.gov/. Follow the directions to complete the TSR and fax it to the telephone number listed on the form.

A Delegated Procurement (DelPro) is used by an Administrative Officer (AO) to purchase telephone services and products (i.e. new telephone lines, new telephone equipment, etc).
Contact your AO for service request approval of this type.

Telephone Repair

To reach the Trouble Report Desk dial 301-402-9935. It is staffed locally Monday through Friday, from 8 a.m. to 5 p.m. except on holidays. Holidays and after-hours calls are forwarded to the NIH Telecommunications Switching Control Center.
