• Obtain a NIH Form 2619 (7/99) http://forms.cit.nih.gov/adobe/procurement/NH2619.PDF
• Telegrams should be typed using ALL CAPITAL LETTERS and double spacing.
• Completed forms must be signed by an approving official
• Forms must be delivered to the following address:

               Bethesda, MD 20817

Note: Include Telex Number if known. If transmission is urgent, call 301-496-4516.

After Hours:
For telegrams & cablegrams, call Center for Information Technology, Telecommunications Telegram Office (301-496-4516), give your name, organization, CAN number, and office telephone number. All bills are to be sent directly to:
