Sample Performance Appraisal Plans
- Most of these sample plans were developed in CY 06 and some language may have to be updated to address CY 08 goals. We are in the process of updating all of the sample plans to ensure they are on the latest version of the PMAP form and to add additional metrics where necessary.
- The HHS PMAP policy has been updated to encourage the documentation of the distinctions between the Fully Successful Level and the Exceptional level. Some sample plans have been drafted to include this distinction. However, this is not a mandatory requirement for CY 2008.
- All PMAP plans must have at least one individual performance outcome (under Section II.B.) that aligns or tracks back to the HHS Strategic Plan. The “cascade” element should be identified in the following way under the appropriate outcome in the performance plan: “This element also relates to and supports objectives in the HHS Strategic Plan, specifically [cite the specific objective].”

- Please use these samples as guidance and then customize your own plan using the standard Performance Plan Form.
