Completing the SF-135:
Agency Contact Information

The first section of the Standard Form 135 (SF-135), Records Transmittal and Receipt, includes contact information for both the agency and individuals initiating the transfer of records as well as the facility to receive the records.



Items 1-5 on the Standard Form (SF) 135
Text color is for instructional purposes only.

Item 1
This item should include the address of the federal records center (FRC) to which you are sending the accession. If using the Washington National Records Center (WNRC), you may simply type the center’s e-mail address, suitland.transfer@nara.gov, followed by "Washington DC – Stop 386" in this section instead.

Item 2
This item should include the name and title of the person authorizing the transfer of the records. For you, this is always the NIH Records Management Officer (RMO). NIH does not require original signatures. In the space provided, include the date the form is filled out in the MM/DD/YYYY format.

Item 3
This item should include the name, e-mail address, and telephone number, including area code, of the person to contact concerning the records. This is your IC Records Liaison. Also include the building number, room number, and CAN number.

Item 4
Leave blank. This item is reserved for use by the WNRC. An FRC staff member will fill in this item upon receipt of the records.

Item 5
This box is for the complete mailing address for the NIH RMO.
