SF-135 Submission Process
After you have filled out the Standard Form 135 (SF-135), Records Transmittal and Receipt, the form must be approved by the appropriate individuals. The SF-135 submission process includes the following steps:
- Electronically submit the SF-135 and a box
list or a box/folder list to the IC
Records Liaison.
- The IC Records Liaison reviews the documents and forwards them to
the NIH
Records Management Officer (RMO) via e-mail.
- The NIH RMO reviews the documents and submits them to the federal
records center (FRC) by e-mailing them to suitland.transfer@nara.gov.
- The FRC reviews the documents for completeness and accuracy. If
acceptable, the FRC will return a copy of the approved SF-135 to the
NIH RMO within 10 working days of original receipt. The approved SF-135
authorizes shipment of the records.
- The NIH RMO provides the IC Records Liaison with a copy of the approved
SF-135.
- The IC Records Liaison arranges for the physical
transfer of the accession to the FRC. This shipment must take
place within 90 days of receipt of the approved SF-135. A copy of
the approved SF-135 and box list are placed in the first box of the
accession.
- After the boxes have been transported to the FRC and shelved in
the facility, the FRC will send the NIH RMO a completed
SF-135. This version of the form will include the name and title
of the person who received the records at the FRC as well as the location
of the records in the storage facility. The FRC will maintain a copy
of the completed SF-135 at their facility.
- The NIH RMO maintains the completed SF-135 in the office and provides
the IC Records Liaison with a copy.
